The macula is a small central portion of the retina which is necessary for sharp “straight ahead” vision needed for reading, driving, or recognizing faces.

There are two types of age-related macular degeneration: Dry AMD, accounts for 80% of all cases of AMD, and Wet AMD, which occurs in the other 20% of cases. Dry AMD is an early stage of the disorder and may result from aging and thinning of the macula. Through a period of years, slow progression to late-stage atrophy can occur and can cause severe vision loss. With Wet AMD, new abnormal blood vessels grow beneath the retina and leak blood and fluid. Eventually if left untreated, the process creates scarring and vision loss. Risk factors for macular degeneration include: Age; Hereditary; Smoking; and Hypertension.

No FDA-approved treatments are available for Dry AMD, although a few are now in clinical trials. A clinical trial called Age-Related Eye Disease Study (AREDS) has produced strong evidence that certain nutrients such as beta carotene, Vitamin C and E, Lutein, and Omega 3-fatty acids may help to slow the progression of Dry AMD. In Wet AMD, Anti-Vascular Endothelial Growth Factor (VEGF) drugs such as Lucentis, Avastin, Macugen, and Eylea can be injected into the eye to cause the abnormal leaking blood vessels to stop bleeding or leaking.

is used to screen for Macular Degeneration. The test involves looking at a square grid with a dot in the center. The grid should be held 12 inches away from the face, while wearing reading eye glasses. Cover one eye at a time and stare at the dot in the center of the grid. If there appears to be any blurry spots, or lines look distorted there could be the possibility of AMD and a phone call to the Retina Department at Katzen Eye Group should be made.

<div class="separator" style="clear: both; An estimated 2.25 million Americans have glaucoma and half are unaware they have it! Damage may be prevented if caught early and treatment is started.

Who gets Glaucoma?

Anyone can get Glaucoma. Main risk factors include: Older age, high eye pressure, family history, African/hispanic ancestry, steroid use, and trauma/injury to your eye.

Are there different types of glaucoma?

Yes, there are 2 general types:

1.Open angle Glaucoma

2. Narrow/Closed angle Glaucoma.

his can only be determined by careful examination by your eye doctor.

Does my eye pressure have to be “high” to have Glaucoma?

No. While the higher the eye pressure, the greater the risk for Glaucoma, Glaucoma can happen at ANY pressure.

How will I know if I have Glaucoma?

Most people (90%) do not have any symptoms of Glaucoma until significant damage has occurred or the eye pressure is very high (10%). Glaucoma is nicknamed the "silent thief of sight" because the loss of vision normally occurs gradually over a long period of time and is often only recognized when the disease is quite advanced. This is why being examined by your eye doctor is so important.

What can be done if I have Glaucoma?

There is currently no cure for Glaucoma; however, medication or surgery can slow or prevent further vision loss. Early detection is vital to stopping the progress of the disease.

Who should have their eyes examined?

• Anyone with a family history of Glaucoma

• African ancestry > 40 years

• All individuals > 60 years

• Anyone with poor peripheral (side) vision

Fortunately, Glaucoma is highly treatable if detected early. The key to preventing serious vision loss or blindness from Glaucoma is annual, dilated eye examination. Your eye doctor will check your vision, eye pressure, and do a complete eye exam. If necessary we will perform additional tests such as visual field testing and optic nerve imaging to help in the diagnosis and monitoring. Contact Katzen Eye Group today to discuss your Glaucoma treatment options.

BOTOX

In the battle against the effects of aging, BOTOX® Cosmetic has become one of the most popular available options. Simple, non-invasive and effective, BOTOX® produces quick, long-lasting results in a single treatment.

How BOTOX® Works

The lines that appear on your forehead and between your eyebrows are usually caused by the tightening of muscles due to stress or simply habit. After years of stress-related frowning, these lines can become deep furrows. In many cases, these lines make you look older than you are, or make you look perpetually angry or worried. Fine lines and wrinkles can also develop around the mouth through a similar process. These lines are especially common with smokers, who purse their lips repeatedly while smoking.

BOTOX® is administered in small doses directly into the muscles that cause these furrows to appear. The muscles become relaxed, easing frown lines and brow wrinkles. Referred to as presbyopia, this process usually begins to create noticeable vision changes between the ages of forty and fifty, and is the result of a loss of flexibility in the lenses of the eyes. At Katzen Eye Group, in the Baltimore, Maryland area, we help treat these changes in vision with monovision LASIK vision correction.

What Is Monovision?

Many people experiencing presbyopia use bifocals—glasses with lens insets in the bottom to correct close vision, while the rest of the lens is structured to aid distance vision. Another way to treat presbyopia is through monovision.

Monovision can be achieved with contact lenses or with corrective surgery such as LASIK. Rather than having multiple lens prescriptions in a single pair of eyeglasses, monovision results in having one eye corrected so that its close distance vision is more efficient, while the other eye is adjusted for distance vision. By using one eye or the other, you can focus on close items or items in the distance. Though it sounds uncomfortable or difficult, it generally only takes a short amount of time to adjust to.

Possible Disadvantages of Monovision

In general, monovision is a comfortable and convenient way to treat presbyopia without having to resort to the inconvenience of glasses. In some cases, monovision treatment results in a slight loss of depth perception, since the eyes are optimized for different vision distances. There may be some blurriness in the non-optimized eye, as well, much like trying to view items at a distance through the section of your bifocals that are meant for close vision.

For additional information about monovision LASIK and how it can help improve your vision, please contact Katzen Eye Group in Baltimore, Lutherville and Towson, Maryland to schedule a LASIK consultation.

Did You Know that Eating Healthy is
Good for Your Eyes?

When your mom told you that carrots are good for your eyes, she was giving you good advice. More than 10 million Americans have Age-Related Macular Degeneration. It is applied with a sterile applicator to the lash line of the upper lashes—never the lower lashes. The medication will reach the lower lashes as you blink, so it does not need to be applied directly to the lower lash line. Latisse is usually applied once a day, in the evening. The Baltimore, Maryland area eye surgeons at Katzen Eye Group can provide additional information about Latisse and how it can help restore your lashes.

Latisse does not provide instant results. It usually takes at least two months before the lashes begin to thicken, with full results occurring within three to four months. In order to see full results from Latisse, you must be diligent in applying it nightly. If you stop using Latisse, your eyelashes will in time return to their previous thickness. After three or four months, at which time your lashes should be noticeably fuller, your doctor might scale your dosage back to once every other night instead of every night.

Be sure to closely follow the instructions provided when using Latisse. Latisse is a prescription medication, derived from a medication used to treat glaucoma. It can cause changes to your internal eye pressure, so it is important not to change your dosages of Latisse. Increasing the dosage will not speed the results, and introduces additional risks that are not present if you apply Latisse according to directions.

If you would like to find out more about Latisse and what it can do for you, please contact the eye surgeons at Katzen Eye Group in Baltimore, Lutherville and Towson, Maryland.

Glaucoma

Glaucoma is the leading cause of permanent blindness worldwide, and affects approximately four million people in the United States. Regular screening is vital to detect this disease and ensure proper treatment is provided early in its progression. Katzen Eye Group in Baltimore, Maryland provides glaucoma screening and has a glaucoma specialist on staff to ensure the highest quality treatment.

What Is Glaucoma?

Glaucoma occurs when the aqueous humor, or the liquid inside the eyeball, builds up or does not drain properly, creating excess pressure within the eye. Eventually, this pressure causes permanent damage to the optic nerve, resulting in vision loss.

The exact cause of glaucoma is not known, but the liquid usually builds up when too much is produced or when the regular drainage system becomes blocked. Under normal conditions, the fluid is replenished regularly and drains out of the eyeballs into the bloodstream. When this process is disrupted, glaucoma results.

Diagnosing Glaucoma

Glaucoma is diagnosed by measuring the pressure inside the eyeball. A procedure called tonometry tests intraocular pressure to see if it is higher than normal. A visual field test is also often administered to test peripheral vision. One symptom of glaucoma is gradual loss of peripheral vision, so the visual field test can determine if your peripheral vision is normal or if it has degraded.

Everyone should be tested for glaucoma during their regular yearly eye exam. If you are over forty or have other risk factors for glaucoma such as diabetes or steroid use, you should have your eyes checked more often. If you start today, you could be doing that in eight weeks and still be only half-way to your final exciting results.

Latisse is a fairly new product that we are now offering, FDA-approved for enhancing the eyelashes. It is easy to apply. It comes in a small bottle and with a supply of disposable applicators. Simply put one drop on a new applicator and draw it along the upper eyelid at the base of your eyelashes. Blot any excess, throw away the applicator, and repeat the process on the other eye with a fresh applicator.

Latisse works by extending the growth phase of the hair growth cycle and by stimulating each hair follicle to produce more than one lash. It also stimulates melanin production although exactly how it works is not fully understood. It is based on some of the eyedrops used for Glaucoma which have a side effect of lengthening the eyelashes.

After eight weeks, you will see longer and darker eyelashes and after 16 weeks, the full treatment period, you will surely be pleased and excited about the results.

There are a few little side effects but most people do not experience them. You might have some itchiness or irritation in your eyes or some redness in your eyes or eyelids. It will most likely be temporary but will certainly stop if you stop using Latisse. Do not use Latisse on the lower eyelids because it could cause extra hair growth beyond your eyelashes.

As with all products, as long as you follow the directions, you will get excellent results.

If you would like to schedule an appointment and learn more about Latisse, please contact our Baltimore eye care office today. By definition, cataracts are the clouding of the eye lens to the extent that they affect vision. By the age of 80, more than half of all Americans have cataracts.

The elderly patients at Katzen Eye Group in the Baltimore area often have questions about the causes of cataracts when they begin hearing about friends and family members having cataract surgery. However, age-related cataracts are only one type. The others are:

Secondary Cataracts - can form after surgery for other ophthalmic diseases

Traumatic Cataracts - can develop after an eye injury

Congenital Cataracts - also known as "infantile cataracts," these cataracts are present at birth

Radiation-inducted Cataracts - can form months and years after radiation exposure

The Katzen Eye Group in Baltimore and Lutherville, Maryland offer cataract treatment for people of all ages. Please schedule an appointment today so we can help you understand your cataract removal options.

Injectable Fillers

When skin loses fluids, wrinkles form. Although the idea that vision improvement could be made possible by flattening the cornea was proposed a century ago, it wasn’t until the 1930s that doctors tried cutting the cornea to change its shape.

Radial Keratotamy - the Predecessor to LASIK

At times, medical discoveries come about in the oddest of ways. A young physician leaves out a Petri dish while studying bacteria and voilà - Penicillin is born. Clinical trials on the drug Sildenafil don't produce anticipated results for use on angina and hypertension, but when the male trial subjects are reluctant to return the unused pills that leads to the discovery of Viagra - an oral drug for male impotence. The story behind Radial Keratotamy is no less amazing.

A Russian doctor had been treating a young lad with cuts in his eyes from broken spectacles during a fall. The boy sustained only minor eye damage when the broken glass shaved a layer off the eye's outer surface. When the previously near-sighted boy showed vision improvement, Dr. Svyatoslav Fyodorov became intrigued, eventually going on to publish his discoveries. American doctors later expanded upon Dr. Fyodorov's discovery after obtaining research funding for the procedure.

A Brief LASIK Timeline in America

1978 - American doctors realize the potential for coupling the Radial Keratotamy surgery with an Excimer laser

1980 - the Excimer laser is first used by a doctor on other biological tissue

1983 - a landmark paper is published in the American Journal of Ophthalmology, outlining the potential for the Excimer laser use in refractive surgeries

1989 - first patent for LASIK granted

1992 - ten surgeons test the Visx laser at ten U.S. centers

2000 - an estimated one million people undergo vision correction with LASIK

Please contact us if you'd like to consider LASIK. For that reason, Baltimore area ophthalmologists at the Katzen Eye Group recommend having your eyes tested and examined every two years if you are under the age of 60, and every year thereafter. You'll want to be tested more frequently if you have one or more of these glaucoma risk factors:

Diabetic or diagnosis of high blood pressure

Family history of glaucoma

Over the age of 60

Steroid user

Eye injury patient

Severely near-sighted

African-American or older aged Hispanic

Common Types of Glaucoma and their Symptoms

Primary open-angle glaucoma and acute angle-closure glaucoma are the two most familiar types of glaucoma. Yet, if your eyelids hang down over them and you constantly joke about the bags under your eyes being large enough to carry groceries in, then it's time to make a change. The surgery you need is called Blepharoplasty, or more commonly eyelid surgery. However, this type of surgery pertains to more than just the eyelids; it can include all the delicate tissues surrounding the eyes that droop, sag or sink as you age.

When it's time to give your eyes a more youthful appearance by tightening the skin around those "windows to your soul," you may think a general plastic surgeon is the best person to call. However, an ophthalmologist with a specialty in Oculoplasty has the highest level of expertise and advanced training to work with these delicate tissues that are so important to your vision.

Blepharoplasty – Knowing Who to Call

Taking any risks with your eyes is never advised, so seeing an ophthalmologist who has the additional training as an oculoplastic surgeon is your best move. These specialists are trained to perform reconstructive surgery on the following conditions:

Aging eyelids or ptosis

Tear drainage problems

Eyelid malposition

Skin cancers of the eyelid

Eye socket problems – also called the orbit area

Anophthalmia – loss of an eye

Oculopastic surgeons at Katzen Eye Group in Baltimore and Lutherville, Maryland are better trained and qualified to perform your blepharoplasty procedure. If you are ready to revitalize your appearance and want the best care for your eyes, then please schedule an appointment today with Katzen Eye Group.

Cataract Surgery Home Care and Complications

Choosing the doctors at Katzen Eye Group for your cataract procedure was your first step toward a successful surgical outcome. LASIK Touch Ups

The lasting effects of LASIK have been studied since it gained widespread acceptance for vision correction in the year 2000, making it the most studied elective procedure. Studies have shown that 95 to 98 percent of laser vision correction patients obtain a minimum of 20/40 vision.

Once you’ve obtained that level of vision, can you maintain it? The answer is yes and no. Let’s look at what the typical LASIK surgery patient can expect as he or she moves immediately from post-op and on down through the years.

General Post-Operative Expectations

Three to six months after surgery, most patients find their vision has stabilized and they can now determine what their final vision will be.

Again, by the six month mark, patients should know the cumulative effect of the LASIK surgery and have stable vision. Visual acuity should remain fairly constant for the rest of life, assuming that cataracts or other age-related diseases don’t develop.

As you approach your forties, your near-sightedness will change, as it does even for those who never needed glasses or contacts. At this point, you’ll likely need some type of correction for reading.

Our LASIK surgeons at Katzen Eye Group in Baltimore and Lutherville, Maryland can help you with initial procedures, as well as LASIK touchups. Fortunately, vision can be restored by implanting intraocular lenses (IOLs). If you'd like to regain clear vision by eliminating cataracts and preventing a reduction in eyesight, then cataract surgery should be considered.

Candidacy for IOLs
Several IOL options exist and your candidacy for each may differ from one to the other. However, generally you are a good candidate for intraocular lenses if:

You have healthy eyes, even if you have one or more cataracts

You have no cataracts, but have encountered problems with your near-vision

You have no medical conditions that would prevent an IOL insertion

You have had a prior LASIK procedure, but your eyes are otherwise healthy

Please contact the ophthalmologists at Katzen Eye Group in Baltimore and Lutherville, Maryland who specialize in intraocular lens placement, if you are suffering from cataracts and want to improve your vision.

Intraocular Lenses

Intraocular lenses (IOLs) that take the place of the eye's natural lens have been hugely beneficial to cataract patients, with more than one million implanted each year in the United States. Since the 1970s, when intraocular lenses found widespread acceptance, intraocular lens technology has advanced significantly. A wide variety of IOLs have hit the market to help patients achieve more flexibility in their vision.

Advances in Intraocular Lenses

From the introduction of intraocular lenses, manufacturers have moved from an inflexible lens material that necessitated larger incisions to acrylic and silicon, which comprise the majority of IOLs placed today. The use of these materials allows the lens to remain soft and pliable so that ophthalmologists can make tiny incisions in which to insert the lens and then unroll it into the eye.

Now, more than ever, you and your doctor have choices in intraocular lenses. Our Baltimore area ophthalmologists at Katzen Eye Group can examine your eyes and help determine which of these intraocular lenses best suits your vision needs:

Multifocal Lenses – offering variable distance viewing with greater possibility that glasses or contacts will not be needed. ReZoom™ and ReStor® provide clear vision at all distances and offer greater freedom from eyewear than previous IOLs.

Toric IOLs – reduces or eliminates corneal astigmatism and greatly improves distance vision without the need for corrective lenses. The Staar Surgical IOL and the AcrySof® IQ Toric IOL will help the blurriness at all distances often caused by an ovoid corneal shape.

Accommodating IOLs – offers focusing from near to far, far to near and in distances between

Aspheric IOLs – traditional IOLs are spheric, but this lens is slightly flatter to provide better contrast sensitivity

Filtering IOLs – offers blue light and ultraviolet (UV) light filtering that your natural lens had before it was replaced with an IOL

Monofocal Lenses – the traditional type of IOL offering vision at only one distance that can be augmented by the use of glasses or contacts.

Please contact the ophthalmologists at Katzen Eye Group in Baltimore and Lutherville, Maryland who specialize in intraocular lens placement, if you want to begin seeing more clearly.

Choosing a LASIK Doctor

If heading to the beach makes you cringe because you can’t decide whether to wear your glasses or risk the elements with contacts, you are not alone. That’s one reason why more than one million people turn to LASIK surgery each year to correct their vision.

However some people use a haphazard approach to choosing a LASIK doctor by selecting a surgeon based on cost or through a slick print advertisement. While the LASIK procedure has proven to be both safe and effective, the doctor you select will still be using surgical instruments on your eyes, so choose wisely.

Questions to Ask a LASIK Doctor

After obtaining LASIK testimonials and referrals from friends and family members who have had the LASIK procedure, check the doctor’s credentials, including board certifications. Then ask a few tough questions at the initial consultation to help determine if your doctor has the qualifications and experience to work on your eyes. Some questions you might want to ask are:
1. How many LASIK procedures have you performed in the last 12 months?
2. What percent of your LASIK patients have achieved a vision of 20/40 and 20/20 or better?
3. What percent of your LASIK patients have reported complications (halos, starbursts, dry eye, etc.) six months after surgery?
4. Will you be performing a wavefront technology diagnostic to determine my candidacy for the LASIK procedure?
5. What percent of your patients have needed a LASIK enhancement?
6. What tests will you perform before and after the LASIK surgery?

The LASIK doctors at Katzen Eye Group will take the time to evaluate your eyes and discuss all available vision correction options, including LASIK. If you are ready to have the best vision of your life, please contact the LASIK doctors at Katzen Eye Group in Baltimore and Lutherville, Maryland.
